Wish List Request With Rubric This 2 page form (request and rubric) can be used by any staff person of the North Syracuse Early Education Program to apply for FNSEEP to purchase item(s) they believe would be beneficial to their classroom, department, overall program, or particular student(s). Decisions are made at monthly FNSEEP meetings by the group present according to scoring of the rubric. This form was last updated August 2018.

iPad Borrower AgreementThis is the iPad lending agreement between Friends of NSEEP and Main Street educators. It was last updated August 2018. Please use this form anytime you are borrowing an iPad.
Reimbursement Request Form This form is to be completed to request FNSEEP reimburse a purchase made for FNSEEP or the NSEEP. Please ensure that you have approval BEFORE you make a purchase you hope to have reimbursed. Obtain a tax exempt form from the FNSEEP treasurer before making any purchases. FNSEEP is a 501(c)(3) and is exempt from New York State tax. Attach all requested documentation and submit to FNSEEP treasurer.
